2013 Community Health Needs Assessment Report

A key provision of the Patient Protection and Affordable Care Act (PPACA) imposes additional requirements on charitable hospital organizations treated as tax-exempt organizations.

Each hospital facility is required to conduct a Community Health Needs Assessment (CHNA) at least every three taxable years and adopt an implementation strategy to meet the community needs identified through that assessment process. 

In 2013, a Community Needs Assessment was completed by OSF HealthCare. 

After the data was collected each community's health needs were prioritized by greatest need.  Administrative and caregiver teams at OSF work together to further support current community-based health programs and services as well as starting new programs to help meet the needs for each community.

Topics identified to be the priority in Knox and Warren Counties were:

  • Access to health services
  • Diabetes/obesity
  • Healthy behaviors
  • Mental health/substance abuse

In 2014, OSF Holy Family Medical Center addressed each of these priorities.

Access to Health Services

  • Investigated the opportunity for a 211 service in the county.  Work still continues and funding opportunities are being investigated.
  • Provided four flu clinics at local businesses and provided over 135 vaccines.
  • Provided a monthly eBlast to over 250 individuals, offering general health and wellness information.
  • Provided assistance in filling over 1000 prescriptions for uninsured patients.
  • Provided Medicare Counseling to community.
  • Provided Affordable Care Act counseling to community.

Diabetes/Obesity

  • Provided a monthly diabetic support group. Number of participants continues to grow.
  • Provided two Nutrition Label Reading classes in cooperation with the local grocer.
  • Provided over 250 A1C screenings in the community.
  • Offered a free eight week health and wellness series.
  • Provided a four part lunch and learn program, free to community members.  The focus healthy eating, lifestyle, and overall health and wellness.  This was offered in conjunction with University of Illinois Extension educators.

Healthy Behaviors

  • Participated in four local health fairs.
  • Provide a bi-monthly program, Stars 55, focusing on health and wellness information to community members age 55 or older.
  • In cooperation with our local health department, we provided hand hygiene education to over 450 Kindergartners and first graders in two school districts.
  • Provided two stroke awareness programs to the community.
  • Provided the Warren County Wellness Program; a free eight week wellness program that includes free screenings, education opportunities and a children’s program.

Mental Health/Substance Abuse

  • Provided two Grief Series sessions, providing an opportunity for grieving members in the community.  It is a free six week series.
  • Held two roundtable sessions with area school counselors to find needs that we could possibly help meet. 
  • Provided OSF Resource Link presentation to all area teachers at their annual in-service, so teachers are aware of the services available to the children they teach.
