How Do I Register as a Patient at OSF Holy Family?
Whether you are an inpatient or outpatient, you will need to register prior to treatment. You will be given your patient care number either through Outpatient Admitting, Admitting or through the Emergency Department. Please have the following information available upon check-in:
- Patient's Social Security number
- Patient's date of birth
- Your insurance or Medicare card including billing address and policy number
- Guarantor (the person responsible for paying the bill) information including address, Social Security number and date of birth
- Power of Attorney/Living Will information
- Physician's orders if you are coming from a doctor's office
Can I Make an Appointment for a Regular Check-Up at OSF Holy Family?
OSF Holy Family Clinics offer non-emergency family and specialty health care.
Appointments can be made by calling (309) 734-1414. Walk-in patients are also welcome.
Where Do I Park?
Visitors
Parking for visitors is located on the west side of the building.
OSF Holy Family Clinics
Patients of the OSF Holy Family Clinics may park on the north side of the building, conveniently located near the clinics entrance.
What is the Hospital's Smoking Policy?
In order to provide a smoke-free environment for our patients, employees and guests and to promote good health, smoking is not permitted by anyone on hospital grounds.

OSF Home Care Services offers full home health care for patients and families in the Monmouth community.
