How do I register as a patient at OSF Holy Family Medical Center?

Whether you are an inpatient or outpatient, you will need to register prior to treatment. You will be given your patient care number either through Outpatient Admitting, Admitting or through the Emergency Department. Please have the following information available upon check-in:

  • Patient's Social Security number
  • Patient's date of birth
  • Your insurance or Medicare card including billing address and policy number
  • Guarantor (the person responsible for paying the bill) information including address, Social Security number and date of birth
  • Power of Attorney/Living Will information
  • Physician's orders if you are coming from a doctor's office

Can I make an appointment with a doctor for a regular check-up at OSF Holy Family?

Yes, we have a 24-hour clinic, which is part of the OSF Holy Family Clinics. The OSF Holy Family Clinics offers non-emergency family and specialty health care. Appointments can be made by calling 309-734-1414. Walk-in patients are also welcome.

I am coming to visit a patient, where do I park?

The Visitor's Parking Lot is located on the West side of the building. Guests who plan to visit long-term care patients on F-wing will find convenient parking on the east side of the building.

I have an appointment at the OSF Holy Family Clinics, where do I park?

There is ample parking for OSF Holy Family Clinics' patients on the northwest side of the building.

I am coming to see a physician at one of the Specialty Clinics, where do I park?

Most of the specialty clinic offices are located on the north side of the building and there are parking spaces conveniently located near the entrances to the specialty clinics. Enter the Visitor/OSF Holy Family Clinics Parking Lot on the west side of the building and follow the signs to your provider's office.

What is the hospital's smoking policy?

In order to provide a smoke-free environment for our patients, employees and guests and to promote good health, smoking is not permitted by anyone on hospital grounds.

Does OSF Holy Family Medical Center have a Home Health Care program?
