OSF Diabetes Care

Thirteen percent of the adult US population has diabetes.  Poorly managed diabetes is the leading cause of adult blindness and end-stage renal disease.  It doubles the risk for heart disease and stroke.  Learning to manage your diabetes lowers your risk for complications from high blood sugar.

OSF Diabetes Care at OSF Holy Family offers a comprehensive program designed for the newly diagnosed or those who have lived with diabetes for a long time.  OSF Holy Family is committed to improving diabetes care and outcomes through education and empowering persons with diabetes to understand and manage their disease.  

unnamed file The American Diabetes Association recognizes this educational service as meeting the National Standards for Diabetes Self-Management Education and Support. 

Our compassionate team members, are Certified Diabetes Care and Education Specialists, trained to help persons with diabetes and their families understand diabetes and meet the challenges of living with diabetes.  A provider referral is needed to schedule an appointment.

Location & Hours

The Diabetes Care office is located in the Rehabilitation and Specialty Clinics Wing on the north side of OSF Holy Family.  To access this wing please use the driveway on the west side of OSF Holy Family and continue around to the north side of the building. 

Mission Partners are available 8 a.m. to 4:30 p.m. Monday through Thursday. Evening hours by special arrangements.
